Overview
Licensed Practical Nurse (LPN) – Long-Term Care unit, Utica, NY. Travel assignment starting 09/14/2026 for 13 weeks, 37.5 hours per week guaranteed, shift 2:30pm–10:30pm (8 hours). Setting: Skilled Nursing Facility with a 40-bed LTC unit. Weekly pay range: $1,449–$1,512. This assignment requires LTC experience and active New York LPN licensure, with local candidates preferred.

Responsibilities
- Administer prescribed medications and treatments in accordance with physician orders and facility policy
- Monitor patients’ status, document vital signs, symptoms, and responses to care
- Assist with activities of daily living and mobility, ensuring patient safety and comfort
- Maintain accurate and timely nursing notes in PointClickCare EMR
- Collaborate with RNs, therapists, and interdisciplinary team to update and implement care plans
- Report changes in condition to the supervising nurse promptly; participate in care plan reviews
- Implement infection control and safety protocols; maintain a clean, organized patient environment

Requirements
- Active LPN license in New York; Nursys license verification required
- Current BLS certification
- 1+ year of LTC experience
- EMR experience with PointClickCare
- Driver’s license; address matching demographics; ability to meet local rate criteria (less than 75 miles)
- RTO (time off) requests must be included at submission
- Ability to float within the same building to support unit coverage
- Education: not specified beyond standard LPN qualification; documentation of relevant LTC experience required
